چکیده

Methods We have designed a cluster trial with a stepped-wedge design (SWD) to test an intervention to increase the uptake to education. This study will include general practices of varying size. There is no published guidance on powering a cluster SWD with varying cluster size. We present the method used to estimate the design effect (DE) which combines the method for calculating the sample size required for a cluster SWD by Woertman et al. with a method for taking into account varying cluster size in a standard cluster trial by Eldridge et al.

The optimal design of stepped wedge trials with equal allocation to sequences and a comparison to other trial designs

Background/Aims We sought to optimise the design of stepped wedge trials with an equal allocation of clusters to sequences and explored sample size comparisons with alternative trial designs. Methods We developed a new expression for the design effect for a stepped wedge trial, assuming that observations are equally correlated within clusters and an equal number of observations in each period b...

Group sequential designs for stepped-wedge cluster randomised trials

BACKGROUND/AIMS The stepped-wedge cluster randomised trial design has received substantial attention in recent years. Although various extensions to the original design have been proposed, no guidance is available on the design of stepped-wedge cluster randomised trials with interim analyses. In an individually randomised trial setting, group sequential methods can provide notable efficiency ga...

Minimum number of clusters and comparison of analysis methods for cross sectional stepped wedge cluster randomised trials with binary outcomes: A simulation study

BACKGROUND Stepped wedge cluster randomised trials frequently involve a relatively small number of clusters. The most common frameworks used to analyse data from these types of trials are generalised estimating equations and generalised linear mixed models. A topic of much research into these methods has been their application to cluster randomised trial data and, in particular, the number of c...

Five questions to consider before conducting a stepped wedge trial

Researchers should consider five questions before starting a stepped wedge trial. Why are you planning one? Researchers sometimes think that stepped wedge trials are useful when there is little doubt about the benefit of the intervention being tested. However, if the primary reason for an intervention is to measure its effect, without equipoise there is no ethical justification for delaying imp...

The stepped wedge cluster randomised trial: rationale, design, analysis, and reporting.

The stepped-wedge cluster randomised trial (SW-CRT) is a novel research study design that is increasingly being used in the evaluation of service delivery type interventions. The SW-CRT is a type of randomised trial in which clusters are randomised to a date at which they initiate the intervention under evaluation. The design involves random and sequential crossover of clusters from control to ...
